Quadricepsplasty and Arthrolysis in Stiff Knees following Patellar Fracture



Abstract
Quadricepsplasty and arthrolysis are techniques devised to release extra-articular adhesions or contracture which are caused by injuries areound the knees leading to fibrosis and scarring of all or part of the quadriceps. Since the earliest
description
of lengthening of the quadriceps tendon by Bennet(1992), several reports have been made by Thompson(1994), Judet(1959), van Nes(1962), Nicoll(1963), Hesketh(1963) and Daoud(1982).
The autors analyzed the results of the Judet¢¥s quardricepsplasty and arthrolysis which were done from June 1990 to October 1993 in stiff knees following following patellar fractures.
Among 11 cases, 9 cases of the patients were followed up for average 22.6 months.
@ES The results were as follows.
@EN 1. Of 9 cases, 7 cases were men and 2 cases were women.
2. Age ranged from 20 to 54 average being 38 years old.
3. Average follow up period was 22.6 months ranging from 12 months to 48 months.
4. The3 initial injuries were femoral fractures with patellar fractures in 7 cases and patellar fractures in 2 cases.
5. The mean preoperative range of motion was from 8.9 degrees of flexion contracture to 46.1 degrees of further flexion and at the most recent follow up, the mean range of motion was from 5.5 degrees to 104.4 degrees. So flexion agin was 58.3
degrees
and extension gain was 3.4 degrees on the averge.
6. By Judet¢¥s criteria of evaluation 6 cases967%) shwed excellent result and 3 cases(33%) showed fair.
7. The patellar fracture seems to be one the causes of stiff knee and the result was less favorable in the stiff knees with comminuted patellar fractures. And it is difficult to treat stiff knee, if intra-ardhesions cannot be released by
arthrolysis.
